- We may submit claims directly to your insurance company. Patients remain responsible for any copayments, coinsurance, deductibles, or other amounts not covered by their plan.
- If our office is considered out-of-network with your insurance provider, you may be responsible for charges not covered by your policy.
- A current copy of your insurance card may be requested in order to verify coverage and billing information.

- If you need to cancel or reschedule an appointment, please contact our office as early as possible.
- Appointments missed without prior notice may be subject to a $40 No-Show fee.
- Late cancellations may also be subject to a fee depending on the circumstances and timing of the cancellation.
- Repeated missed appointments or frequent late cancellations may affect your ability to continue receiving services from our practice.
- Appointment reminders may be sent by phone, email, or text. Patients are still responsible for keeping scheduled appointments even if a reminder is not received.
- Regular follow-up appointments are required in order to continue receiving prescription medication refills.
- Prescription refill requests are not processed as emergency requests and are generally handled during normal business days.
- Please request medication refills before your current supply runs out and contact your pharmacy first to confirm whether refills are already available.
- Telemedicine appointments are treated with the same importance as in-person visits. Patients should participate from a quiet, private, and appropriate location.
- Medications and relevant medical information should be available during the appointment. For minor patients, a parent or legal guardian may be required to participate.
- Failure to follow telemedicine guidelines may result in the visit being discontinued or recorded as a missed appointment.
- Our Patient Portal provides a secure and convenient way to complete forms, review billing information, request prescription refills, and communicate with your provider.
- New patients may receive an email invitation with instructions for creating their portal account before their first appointment.
- Patients are expected to communicate respectfully with providers, staff members, and other individuals involved in their care.
- If you are unable to attend an appointment, please contact our office in advance whenever possible.
- If you are experiencing a medical or psychiatric emergency, or believe you may harm yourself or someone else, call 911 or go to the nearest emergency room immediately. Our office is not an emergency or crisis center.
- Please allow appropriate processing time for prescription refill requests, insurance updates, forms, and other administrative matters.
- Questions or concerns regarding your care or office policies should be communicated respectfully to our staff or office management.
- Threatening, abusive, disruptive, or inappropriate behavior may result in termination of services.
- Services are generally provided to patients located in Florida and may require verification of Florida residency.
- Patients who maintain residences in multiple states may be asked to provide documentation confirming their Florida address.
- Temporary travel outside or within Florida may affect the availability of certain services, particularly telemedicine appointments.
- If you permanently relocate outside Florida, continued care will depend on whether your provider is legally authorized to practice in your new state.
